Supportive Clinical Documentation

At Crescent Counseling Center, we understand that clients may occasionally require documentation to support emotional, academic, or occupational needs. Our clinicians offer supportive clinical Documentation as an optional service for current clients, when clinically appropriate.

This may include:

  • Emotional Support Animal (ESA) letters

  • Letters for academic accommodations (e.g., extended time, reduced course load)

  • Workplace support documentation (e.g., flexible scheduling, mental health leave)

  • General letters of support related to mental health and functioning

These letters are provided only after a clinical relationship has been established and sufficient information is available to support a professional recommendation. Documentation is based on the clinician’s judgment and must align with ethical and legal standards.

Important Guidelines:

  • Documentation is not guaranteed and may require multiple sessions or assessments.

  • We do not provide documentation for legal, disability, or forensic purposes.

  • A minimum of one month of consistent treatment is typically required before considering documentation requests.

  • Fees may apply for the time involved in preparing letters, and this service is not billable to insurance.
